We are a telemedicine clinic; we do not have an IN OFFICE experience. If you require more Care or attention than we can provide, we may refer you to another Provider and clinic. This includes but is not limited to the following:

  • Symptoms, conditions, or diagnoses we can not treat over telehealth platforms via messaging, patient portals, and telehealth visits.

  • Help navigate the communication platforms, i.e., patient portal, secure and nonsecure messaging, and links to fill out paperwork.

  • Unable to Handle and administer medications at home

WHAT IS INCLUDED IN MY COSTS OF THERAPY?

All medications include management of your treatment. Management includes the time and staff it takes to create or change a treatment plan, process, and order medications, unlimited messaging through the portal and text line(s), and also includes up to max of one 15-minute follow-up phone call and 60 min telehealth visit with the Provider to review labs and/or make changes to your treatment plan. If you request additional visits with The Provider, costs are $50 for 30 minutes and $100 for 60 minutes. In addition, we recommend utilizing your patient portal for non-urgent questions.

WHEN CAN I START SEEING RESULTS?

On average, it takes at least two months to start seeing and feeling the full benefits of hormone replacement therapy. Please allow your body to adjust to the changes and do not deter from the treatment plan. Please contact us in your patient portal if you have concerns about your treatment plan. We want to help ensure you feel entirely comfortable with your treatment.

WHAT MAKES US DIFFERENT FROM PRIMARY CARE?

We specialize in hormones. We believe in optimizing lab values, and how we care for you is based on how you are feeling and your symptoms, not lab values. Primary Care does not practice medicine like this. Instead, we practice based on current research and years of experience. We are happy to provide research studies to corroborate our treatment plans; we have several links to peer-reviewed articles and studies on our website, www.dialnhealth.com See ‘References’ tab.

CO-MANAGING POLICY

DialN Health, LLC and CroninDialN Health PLLC will not co-manage hormone medications with another provider, including thyroid and weight loss therapy. Please inform your primary care provider about your new medications, but they will likely not be receptive to the treatment plan because it challenges how they practice medicine. If you discontinue or change your therapy because “ my doctor said…” we will immediately terminate your therapy and defer you to your primary care provider to take over your Care.

We will not collaborate with another provider to try and change their mind regarding hormone replacement.

FOLLOW-UP VISITS

The text reminder you receive for Follow-up Appointments for labs, refills, or check-ins text reminders will say you have an IN-OFFICE visit. This is NOT an IN-OFFICE visit, and it is NOT a telehealth visit. It’s a reminder that you must complete a form that will be sent to you that day. Once we process the form(s) (1-24 hour processing time), we will contact you with the following steps, lab orders, or changes to your plan. Therefore, it is imperative to get forms done on time or complete any requested information so it does not delay your treatment.

Your follow-up appointments are listed in your care plan below under the section “Plan.”

FOLLOW-UP LAB TESTING

For a Patient to adhere to the treatment recommendations established by the healthcare Provider, lab testing is mandatory. Once the Patient initiates hormone therapy, follow-up lab testing is done at 8 weeks, 20 weeks, 44 weeks, and every 6 months after that or at the Provider’s discretion. This is dependent on what program the Patient is on. When patients are due for their labs, they will be notified via a follow-up intake form.

REFUND POLICY

All sales are final and non-refundable. DialN Health, LLC and Cronin DialN Health PLLC will not refund a purchase because patients are unsatisfied with their treatments, service, or processing times, or their meds have expired before use.

TECHNOLOGY DISCLAIMER 

Our patients must use their phones and email to communicate with us. We try to help our patients navigate the new technology but can only provide so much help. The Patient maintains full responsibility if they refuse to utilize our communication platforms properly and legally.

SUBCUTANEOUS INJECTION:

Subcutaneous injection is in the fat layer of stomach, glute or thigh.

-Preparation of Insulin syringe injection for SQ: 1. Pull off the safety caps on the insulin syringe and pull back on the plunger until you reach {INSERT DOSE IN UNITS} of air into your syringe. 3. Carefully insert the needle into the top of the vial and push air into the vial. 4. Turn the vial upside down and carefully draw the liquid medication into the syringe until you reach {INSERT DOSE IN UNITS} 5. If you see any air bubbles, use your fingers to lightly tap or flick the syringe to draw the bubbles to the top of the syringe near the needle. Now, you are ready to inject. 6. See https://dialnhealth.com/faqs/ for instructions on how to administer a SQ injection. Do not pre-fill your syringes, as the potency of the medicine is unknown after a prolonged period.

OXANDROLONE

-Storage: Keep in a cool dark cabinet. This does not need to be refrigerated.

-Education: Oxandrolone is used with a diet program to cause weight gain in people who have lost too much weight due to surgery, injury, chronic (long-lasting) infections, trauma, or who are underweight for unknown reasons. Oxandrolone is also used to treat bone pain in people with osteoporosis (a condition in which the bones become thin and weak and break easily) and to prevent certain side effects in people who take corticosteroids (a group of medications used to treat many conditions that involve inflammation or swelling of part of the body) for a long time. Oxandrolone is in a class of medications called androgenic hormones. It works by increasing the amount of protein made by the body. This protein is used to build more muscle and increase body weight.


-Possible Side Effects: difficulty falling asleep or staying asleep, enlargement of the breasts, changes in sex drive or ability, swelling of the arms, hands, feet, ankles, or lower legs, new or worsening acne (especially in women and prepubertal men), enlargement of the clitoris, deepening of the voice, increase in facial hair, and baldness(in women), abnormal or absent menstrual periods, erections of the penis that happen too often or do not go away, enlarged penis, pain, swelling, or decreased size of testes, frequent, difficult, or painful urination

BERBERINE

-EDUCATION: Berberine is an herb extracted from several plants, specifically shrubs called Berberis. Berberine originates from an ancient Chinese herb called Coptis Chinensis French. The alkaloid chemical structures of berberine set it apart from other hypoglycemic agents, such as biguanides and sulfonylureas. Berberine is used in traditional Chinese medicine to treat various ailments. Because of its yellow color, it is also frequently used as a dye. Studies have shown that berberine is beneficial for people with a range of health problems, including diabetes and cardiovascular disease. It is also used for metabolic syndromes such as prediabetes, weight loss, dyslipidemia due to its lipid-lowering effect, polycystic ovary syndrome, and other health conditions.
The exact mechanism of berberine on glucose metabolism is still being investigated. The good news is that in both in vivo and in vitro studies, berberine promoted an insulin-sensitizing effect while reducing insulin resistance. Regarding berberine’s lipid-lowering effect, there is evidence that berberine shunts the body’s lipid production through the AMP-activated protein kinase.

NANDROLONE DECANOATE

-Possible Side effects: TRT’s side effects are similar for both Men and Women, with some distinct differences related to sex listed below: Both sexes can experience acne, mood swings, swelling of the neck or limbs, injection site reactions, fluid retention, hair loss, increased red blood cell count, hyperlipidemia, infertility, and increased snoring. Male patients can also experience gynecomastia or breast cancer. Do not take it if you have a history of blood clots. Female patients can also experience deepening of the voice, clitoral enlargement, increased hair growth on the face, and hair loss. There is an increased risk of estrogen-related cancers, including breast cancer, endometrial, and cervical cancer, especially if the patient has an existing family history of risk. Patients using HRT should seek medical attention immediately if a heart attack or stroke symptoms are present. These can include chest pain, shortness of breath, trouble breathing, weakness, numbness, unusual pain or swelling in one part or side of the body, and difficulty speaking.

THYMOSIN BETA 4

-Education: Thymosin Beta 4 is a naturally occurring peptide found in all cells in humans and animals except the red blood cells. It is related to Thymosin which is an important immune system hormone secreted from the thymus gland. TB4 has been used extensively in racehorses to prevent injury and promote healing. It is low in molecular weight, which allows its quick migration to sites in the body that need healing. TB4 is currently in clinical trials for the treatment of HIV, AIDS, and influenza.

How does TB4 work?

It’s largely due to the U.S. Food and Drug Administration (FDA) refusing to acknowledge peptides as legitimate medicine. Nevertheless, the side effects of the peptide remain not serious or damaging to health.

In accordance, the majority of side effects are related to the peptide being ineffective or unable to resist the infection. Overall the Peptide is well tolerated. However, that’s not an awful by-product of something the peptide is attempting to treat.

SELANK

-Storage: Keep in the refrigerator.

-Education: Selank is another ACTH/MSH-like peptide of the melanocortin class most closely related to the analog tufstin. While traditionally prescribed for anxiety and depression, it has been known to be effective in many other treatments related to immune modulation, anticoagulation, PTSD, ADHD, and metabolic syndromes.
Selank has pronounced anxiolytic activity and acts as a stable neuropsychotropic, antidepressant, and anti-stress drug that relieves aggression and fear reactions in different animal species.
Mechanism of action: Selank primarily works by regulating neurotransmitter levels in the brain, including serotonin, dopamine, and gamma-aminobutyric acid (GABA). These neurotransmitters play a its BDNF properties to enhance Gaba play a crucial role in mood regulation, anxiety, and other cognitive processes.

GONADORELIN

-Reconstitution of a 2mg/ml: 1. Wipe the top of the bacteriostatic water vial and gonadorelin vial with alcohol before use. 2. Use the large needle and syringe provided to draw up 5mL of Bac Static water. 3. Once you have drawn up 5mL of BacStat water, insert this needle into the gonadorelin vial and push all 5mL of water into this vial. 4. Once the water is mixed with the med, do not shake, gently swirl to completely mix. 

-Storage: This medication needs to be refrigerated after reconstitution. 

-Education: Gonadorelin helps to prevent testicular atrophy and also helps to maintain fertility

-Possible Side effects: headache; flushing; nausea or abdominal discomfort; dizziness or lightheadedness; pain, swelling, or itching at the injection site; or. skin rash.

TAMOXIFEN

-Storage: Keep in a cool, dry place. This does not need to be refrigerated.

-Education: Nolvadex is a prescription medicine used to treat the symptoms of Breast Cancer. Nolvadex may be used alone or with other medications. 

Nolvadex belongs to a class of drugs called Antineoplastics, Estrogen Receptor Antagonists. 

Men use it to lower their estrogen levels when on Testosterone when they are experiencing side effects of high estrogen. 

-Possible Side effects: vision changes, eye pain, easy bruising or bleeding, mood changes, swelling of the ankles or feet, and unusual tiredness

Low Dose Naltrexone:

-Storage: Keep in a cool, dry place. This does not need to be refrigerated.

Education: FDA-approved medication that has been used since the 1980s for opioid withdrawal. It is used at a higher, standard dose of 50 mg per day. However, low doses of this medication (hence, low dose naltrexone or LDN), have been found to modulate the immune system and have shown promise in improving cases of autoimmune disease. These include Crohn’s, MS, and Hashimoto’s, as well as other immune system-related conditions such as cancer and HIV/AIDS. Doses of 1.5-4.5 mg per day are usually recommended and have been reported to enhance immune function by increasing the body’s production of endorphins, also known as endogenous endorphin production. [1] This is important for autoimmunity because the increased endorphins help to modulate and regulate the immune system. LDN also reportedly works by reducing inflammation, promoting DNA synthesis, and slowing down motility in the GI tract, to facilitate healing. To get a little more technical, LDN balances the immune system by increasing the amount of T-regulatory cytokines and modulating TGF-B, leading to a reduction of Th-17, the promoter of autoimmunity. [3] This means that it turns off the cells that cause autoimmunity!

-Benefits: There is a lot of clinical data that shows reduction in TPO antibodies and TG antibodies, as well as autoimmune symptoms, in about 40% of clients with Hashimoto’s who have tried LDN.

-Possible Side Effects: While LDN appears to be well-tolerated, known side effects of naltrexone include dizziness, headache, difficulty sleeping (insomnia), vivid dreams, nausea, vomiting, decreased appetite, joint pain, muscle cramps, tooth pain, and anxiety. In rare instances, there can be severe side effects such as increased blood pressure or heart rate, confusion, depression, and allergic reactions. Contact your healthcare provider right away if you experience any of these. People with kidney disease or liver disease may need to speak with their doctor before starting LDN. Those who are pregnant or breastfeeding should also speak with their doctor before starting this medication

-Interactions and Risks: Even at low doses, naltrexone should not be taken with certain medications or alcohol. Do not take LDN with opiate painkillers such as Ultram (tramadol) and OxyContin (oxycodone). Naltrexone may interact with chemotherapy drugs and the following medications (known as CYP450 substrates):

Ibutamoren MK 677:

-Storage: Keep in a dry, dark location

-Education: The benefits include helping to improve metabolism and aiding in weight loss. Additionally, Ibutamoren can help to increase muscle mass and strength and has been shown to improve bone density.

-Possible Side Effects: lethargy, severe muscle pain., severe headaches and migraines, reduced sensitivity to insulin, increased blood sugar levels, increased feeling of hunger, cravings for sweets, and joint pain.

Pregnyl:

-Storage: Keep in refrigerator after reconstitution

-Reconstitution:

1. Wipe the top of the bacteriostatic water vial and Pregnyl vial with alcohol before use. 2. Use the large needle and syringe provided to draw up 10ml of Bac Static water. 3. Once you have drawn up 10mL of BacStat water, insert this needle into the Pregnyl vial and push all 10mL of water into this vial. 4. Once the water is mixed with the med, do not shake; gently swirl to mix completely.

Education: Pregnyl is human chorionic gonadotropin (HCG). When you combine hCG with TRT, you don’t experience TRT symptoms like testicle shrinkage and low sperm count. Your testes start producing testosterone naturally, and your serum testosterone level stays normal. TRT without hCG can reduce intra-testicular testosterone levels by 94%. In men, hCG acts like luteinizing hormone (LH). LH stimulates Leydig cells in the testicles, which results in the production of testosterone.

LH also stimulates production of sperm within structures in the testicles called seminiferous tubules. As hCG stimulates the testicles to produce testosterone and sperm, the testicles grow in size over time.

Possible Side Effects: For men, the most common side effects of hCG injections include:

gynecomastia, or the growth of male breasts, pain, redness, and swelling at the injection site, stomach pain, nausea, vomiting. In rare cases, people taking hCG have developed blood clots. Although also rare, allergic reactions can occur, including mild skin rashes and severe anaphylactic reactions. Stop using HCG and get emergency medical help if you have any of these signs of an allergic reaction: hives; difficulty breathing; swelling of your face, lips, tongue, or throat. Call your doctor at once if you have any of these signs of a blood clot: pain, warmth, redness, numbness, or tingling in your arm or leg; confusion, extreme dizziness, or severe headache.

TADALAFIL

-Storage: Keep in a cool, dry place. This does not need to be refrigerated. 

-Education Tadalafil (Cialis) is used to treat erectile dysfunction (ED, impotence; inability to get or keep an erection), and the symptoms of benign prostatic hyperplasia (BPH; an enlarged prostate), which include difficulty urinating (hesitation, dribbling, weak stream, and incomplete bladder emptying), painful urination, and urinary frequency and urgency in adult men. Tadalafil is in a class of medications called phosphodiesterase (PDE) inhibitors. It works to treat erectile dysfunction by increasing blood flow to the penis during sexual stimulation. This increased blood flow can cause an erection.  

Possible Side Effects: headache, indigestion or heartburn, nausea, diarrhea, flushing, pain in the stomach, back, muscles, arms, or legs, cough. Some side effects can be serious. If you experience any of these symptoms, call your doctor immediately or get emergency medical treatment: sudden decrease or loss of vision (see below for more information), blurred vision, changes in color vision (seeing a blue tinge on objects or having difficulty telling the difference between blue and green), sudden decrease or loss of hearing (see below for more information), ringing in ears, erection that lasts longer than 4 hours, dizziness, chest pain, hives, rash, difficulty breathing or swallowing.

MINOXIDIL

-Storage: Keep in a cool, dry place. This does not need to be refrigerated. 

-Education: Oral minoxidil (Loniten) is an antihypertensive agent; topical minoxidil (Rogaine) is used for alopecia. Topical minoxidil is likely effective in producing moderate hair growth in approximately 30% of men and 60% of women with common hereditary hair loss.1  Lesser growth or a halt in the worsening of alopecia are also frequent outcomes.2 Due to its potency and adverse reactions, oral minoxidil is used mainly for patients with severe, drug-resistant forms of hypertension. Tolerance to prolonged therapy with oral minoxidil does not appear to be a problem. Although the oral dosage form was originally approved in October 1979 for use in hypertension, minoxidil was first discovered in 1965. In August 1988 the topical formulation was approved for of alopecia. After declining Upjohn permission to market topical minoxidil as a non-prescription drug in July 1994, the 2% topical solution was subsequently approved for over-the-counter use in men with alopecia in February 1996. In September 1996, Pharmacia and Upjohn petitioned the FDA to increase the topical solution formulation from 2% to 5%; the higher-strength solution has been shown to elicit a more rapid hair growth response (8 weeks vs. 16 weeks) and to regrow an average of 45% more hair than Rogaine Regular Strength.2 The 5% topical solution (Rogaine Extra Strength) for common hereditary hair loss was approved FDA in November 1997. Approval to market 2% topical solution to women was granted October 1996. Minoxidil foam (Men’s Rogaine Foam) was approved for men in January 2006; potential advantages over the solution formulation include the absence of propylene glycol (potential irritant), ability to limit spread of medication, and less time to dry after application.3

Possible Side Effects:

The adverse reaction profile for minoxidil depends upon its use. Systemic adverse reactions are unlikely from topical administration. Placebo-controlled trials with topical minoxidil only showed an increase in dermatological effects from the active drug.

Minoxidil is a peripheral vasodilator. All direct vasodilators produce a marked increase in plasma renin activity, which leads to water and sodium retention and sometimes congestive heart failure. This renin release is believed to be partially mediated by the beta-adrenergic system. The degree of fluid retention is somewhat related to the potency of the vasodilator. Due to its potency, fluid retention (edema) occurs routinely with oral minoxidil and usually requires concomitant administration of a loop diuretic. Without a diuretic, rapid fluid retention can occur within a few days of minoxidil therapy. Temporary edema occurred in 7% of patients who were not edematous when minoxidil was initiated.10  Ascites also has been reported. A restricted dietary intake of sodium can minimize fluid retention and resultant peripheral edema. Rarely, fluid retention is refractory to diuresis and discontinuation of minoxidil is required. Vasodilation may also produce headache.

Minoxidil causes reflex tachycardia; sinus tachycardia may occur. Angina may become apparent, or worsen, secondary to increased myocardial oxygen demand associated with tachycardia and increased cardiac output. Tachycardia and subsequent angina usually can be prevented with the coadministration of a beta-blocker or other sympathetic nervous system suppressant.10

Minoxidil has been shown to transiently lower hematocrit, hemoglobin, and erythrocyte count by approximately 7%. Serum creatinine and BUN also have been shown to increase an average of 6% in patients on minoxidil therapy. Increases in alkaline phosphatase, without other evidence of hepatic abnormality, also has been reported. During the course of therapy, these laboratory abnormalities have been shown to return to pretreatment values. Thrombocytopenia and leukopenia also have been reported.10

Hypertrichosis (elongation, thickening, and enhancement of fine body hair), without evidence of virilism or endocrine abnormalities, is an embarrassing adverse effect that often occurs with oral minoxidil. This effect is usually evident within 3—6 weeks of therapy and occurs on the temples, between the eyebrows, or in the sideburn area. Hair growth also can appear on the arms, legs, and scalp. It is reversible following discontinuation of the drug.10

Oral minoxidil has occasionally been associated with appearance of a bullous rash and Stevens-Johnson syndrome. Topical minoxidil therapy produces local dermatological reactions including contact dermatitis, local burning, pruritus, erythema, or xerosis. Many other adverse effects have been reported during administration of topical minoxidil preparations, but none has been directly attributed to the drug.10

Gastrointestinal adverse effects associated with orally administered minoxidil include nausea and vomiting.10

-Education: Testosterone converts into Estradiol. In some men this can cause Gynecomastia, ED and moodiness. We use a low dose of Anastrozole which is an Aromatase Inhibitor to prevent side effects that can be caused by high estrogen. Men need a healthy amount of estrogen for cellular function. We will use Anastrozole only to treat side effects of high estrogen if they present.
